New Patient Registration
If you live within our practice area and wish to register as a patient you should inform the receptionists and they will give you a health questionnaire to fill in. You should then fill in and sign the purple registration form (GMS1) that you will be given.
All patients will be asked to make an appointment for a new patient health check with the nurse.
As a guide the following roads form the practice boundary:
Folds Road, Tonge Moor Road,Thicketford Road, Crompton Way, Bury Road, Long Lane, Radcliffe Road, Raikes Aqueduct Road, Raikes Lane, Manchester Road, Bradshawgate.
- Asylum Seekers should provide relevant documentation of their asylum status. For example a passport and asylum card for verification.
- Foreign Students should provide relevant documentation such as passport copy and student ID for identification purposes.
- Carers – Do you take care of someone who is chronically ill and is dependant on you for their care? Please ask for a carers information and register as a carer at reception.
